2018-2019 Season Requirements - Physical Exam, Drug Testing, Concussion Baseline
The following email details your responsibilities as a parent of a player for SVHA regarding concussion baseline testing, drug testing, and physical exams.
All players must have a physical examination using the PIAA form (every year), drug test (every year for 7th grade and older), and a concussion screening (every other year).
No players are able to participate in practices, scrimmages, or games without these requirements met.
Requirements:
1. Seneca Valley School District Sport Registration
- Through the SV Portal, you must designate that you are participating in a winter sport, as well as provide electronic signature for waiving liability, acceptance of risk, etc.
- Log in to the SV Portal, select "Sports" section under your student's name and complete these two requirements.
2. Physical Exam
- Obtain your player's physical exam (must be dated after June 1, 2018).
- You can visit your personal pediatrician, family physician, or a physician at an urgent care facility (typically $30 per sports physical, MedExpress sent flyer to sports teams sharing a $30 fee for sports physicals).
- SV Senior High is also offering physical exams at the dates listed below. $20 per exam (cash or check only). No appointments - players get in line when they arrive.
- Please take with you the PIAA CIPPE form, Sections 5 and 6, to be signed by the administering physician. Form available here.
- Please have an exam completed and forms signed by August 15th.
3. Drug Test and ImPACT Baseline Concussion Test
- Please take with you the Drug Testing Consent Form and cash or check for $30. Form available here.
- No form or fee needed for concussion baseline test.
- The following are dates for physical exams, drug testing, and imPACT concussion baseline testing at the Seneca Valley Senior High School:
July 18, 2018 - Drug Test & Concussion Test @ Senior High (3-7pm) Physicals 3:00-7:00
July 19, 2018 - Drug Test & Concussion Test @ Senior High (8am-12pm; 1-3pm) Physicals 8am-12pm Only
July 25, 2018 - Drug Test & Concussion Test @ Senior High (3-7pm) NO Physicals
July 26, 2018 - Drug Test & Concussion Test @ Senior High (8am-12pm; 1-3pm) Physicals 8am-12pm Only
No paperwork to turn in to SVHA with regards to drug testing and concussion testing. In August, I will receive a list of SV Ice Hockey players that have completed their drug testing and concussion baseline.
If you are unable to make any of the above dates, you must schedule a test through the Athletic Department.
Forms: The "Sports" section on your player's SV Portal can be used to obtain all the necessary forms and waivers. Forms can also be found on the SVSD website at https://www.svsd.net/domain/1896
Deadline: August 17th (or any practice in August before this date)
Players turn in their PIAA paperwork (Sections 1, 5, and 6) to the SVHA Registrar or their coach at their team's practice in August. A player will not be eligible to be on the ice if they fail to submit the completed PIAA forms or meet the Seneca Valley School District's requirements with regards to drug testing and concussion baselining.
DO NOT turn in your paperwork to the Seneca Valley Athletic Director - we are a Club Sport, and all paperwork stays within our organization.
If you son/daughter plans to participate in another sport in the 2018-2019 school year, please retain a copy of your paperwork for your records.
